29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.  (a) Coo conference room, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in potential minor burns and electrical shock.   (b) Gym area, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in potential minor burns and electrical shock.   (c) Office 2-013, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in potential minor burns and electrical shock.   (d) Office 1-016, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in potential minor burns and electrical shock.   For Reference:  Underwriters Laboratories (UL) refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.             Equipment:            120-volt 15-amp Relocatable Power Tap           Condition:               A relocatable power tap that is designed to be plugged into a branch circuit was connected to a 120-volt 15-amp 14-gauge extension cord.  The U. S. Forest Service RR-Siskiyou NF was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.303(b)(2), which was contained in OSHA inspection number 1702562, citation number 3, item number 4 and was affirmed as a final order on 29 December 2023, with respect to a workplace located at High Cascades Ranger District.

29 CFR  1910.37(a)(4):Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) were not in proper working order at all times.  (a) Room I-020, on or about 9 May, 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall  and burn injuries from trying to exit building during an emergency event.  (b)Engineering section, near plotter, on or about 9 May, 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall  and burn injuries from trying to exit building during an emergency event.  (c) First floor restroom, on or about 9 May, 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall  and burn injuries from trying to exit building during an emergency event.  (d) Building back exit, on or about 9 May, 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall  and burn injuries from trying to exit building during an emergency event.  (e) Room 2-124, on or about 9 May, 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall  and burn injuries from trying to exit building during an emergency event.  The U. S. Forest Service RR-Siskiyou NF was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.303(b)(2), which was contained in OSHA inspection number 1702562, citation number 3, item number 1 and was affirmed as a final order on 29 December 2023, with respect to a workplace located at High Cascades Ranger District, 47201 Highway 62, PROSPECT, OR 97536.  The Corvallis Forestry Sciences Laboratory was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.37(a)(4), which was contained in OSHA inspection number1731859, citation number 3, item number 2 and was affirmed as a final order on 22 April 2024, with respect to a workplace located at 3200 Southwest Jefferson Way, CORVALLIS, OR 97331.

(a) Auto shop building, on or about 9 May 2024 and times prior thereto: Employer failed to update Safety Data sheets exposing employees to potential injuries and illnesses.
