On or about May 10, 2024, at the worksite: employees were exposed to a 14 foot fall hazard while installing weather protection building wrap on a 5:12 pitched roof, in that no fall protection was used.

On or about May 10, 2024, at the worksite: an employee was exposed to a 14 foot fall hazard while accessing a ladder that was positioned on a 5:12 pitched roof, in that the ladder was not secured and the surface was not level.
