At the worksite, located at the intersection of Carl Ave and Shirley Ave Belleair, FL 33756; On or about 5/13/24, the employer exposed employees to entrapment and crushing hazards, in that, precautions necessary to protect employees adequately from cave-ins was not provided while employees conducted excavation of class-c soil for storm drain lines to be installed.

At the worksite, located at the intersection of Carl Ave and Shirley Ave Belleair, FL 33756; On or about 5/13/24, the employer exposed employees to entrapment and crushing hazards, in that, employees conducted excavation operations for storm drain lines to be installed with loose soil left unsecured on the excavation face.
