29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.   On or about February 26, 2024, the employer failed to report the work-related death of an employee within 8 hours to the Occupational and Health Administration (OSHA).  The work-related incident occurred on March 3, 2022, and the employee was pronounced dead at approximately 1444hrs on March 4, 2022.
