29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) 8242 County Road 13, Longmont CO: On or about May 8, 2024, and at times prior, employees were exposed to catapult and subsequent fall hazards of approximately 25 feet while accessing the work location in a JLG G12-55A aerial lift without being secured to the lift.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) 8242 County Road 13, Longmont CO: On or about May 8, 2024, employees were exposed to fall hazards of approximately 120 feet to the ground below, while conducting framing operations trusses without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  a) 8242 County Road 13, Longmont CO: On or about May 8, 2024, and at times prior, employees were exposed to fall hazards while using an extendable ladder that was approximately one foot above the eave height to access and egress upper levels.
