On or about May 15, 2024, at the above addressed jobsite, employees working on the ground, under a worker performing residential roofing operations, were not wearing  hard hats to protect against falling materials and debris.

On or about May 15, 2024, at the above addressed jobsite, an employee performing residential roofing operations, fourteen (14) feet above the ground, was not protected from falls, thereby exposing employee to fall hazards.  b. On or about May 15, 2024, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet one (1) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about May 15, 2024, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards between 14 feet, 0 inches and 9 feet, 1 inch high while performing siding activities on a residential structure without properly using fall protection.
