29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   17602 La Cantera Pkwy, San Antonio, TX 78257: On or about May 17, 2024, and at times prior thereto, employees were exposed to a fall hazard while working on a stack of wooden pallets that was lifted by a JCB Model 506-36 SN 380025 Telehandler.

29 CFR  1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178:  17602 La Cantera Pkwy, San Antonio, TX 78257: On or about May 17, 2024, and at times prior thereto, the employer did not ensure employees operating a JCB Model 506-36 SN 380025 Telehandler were trained prior to use.
