29 CFR 1910.1030(d)(4)(i): The employer did not ensure that the worksite was maintained in a clean and sanitary condition:   a) Soiled Utility Closet: On or about 5/16/24 and at times prior, the employer exposed employees to bloodborne pathogens hazards, in that housekeeping was not provided to the area while employees enter the closet to discard of regulated and biohazard waste.

29 CFR 1910.1030(g)(2)(vii)(D): The bloodborne pathogens training program did not contain an explanation of the employer's exposure control plan and the means by which the employee could obtain a copy of the written plan:   (a) Facility: On or about 5/16/24, the employer exposed employees to biological hazards, in that a written exposure control plan was not readily accessible to employees while they were required to perform bloodborne pathogens work activity such as but not limited to administering medication with use of disposable needles.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure:  (a) Facility: On or about 5/16/2024, and at times prior, the employer exposed employees to biological hazards, in that the hepatitis B vaccination or antibody testing for immunity was not provided to employees during their initial 10 days of occupational exposure while performing nursing duties including removal of bloodborne pathogen material.
