On or about May 21, 2024, at Sawgrass Plantation: employees were exposed to 9- foot 10-inch fall hazards while installing solar panels on a 6/12 pitched roof, in that a fall protection system was not used.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about May 21, 2024, at Sawgrass Plantation: employees were exposed to 9-feet 10-inch fall hazard, in that a Gorilla Multi Position ladder was used and did not extend the 3-feet above the roof landing to access a 6/12 pitch residential roof.
