29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about May 21, 2024, employees performing mechanical repairs to the Plywood Stacker Machine at the Plystack, LLC facility located at 105 N Owen Ave, in Sutton, Nebraska were exposed to amputation hazards in that the employer has not developed and utilized energy control procedures to eliminate residual electrical and pneumatic energy, and the unexpected start up of the machine.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about May 21, 2024, employees involved in plywood weatherproofing at the Plystack, LLC facility located at 105 N Owen Ave, in Sutton, Nebraska were exposed to amputation hazards when working near the   Plywood Stacker machine in the following two instances:  a.  Four separate chain and sprocket assemblies at the end of the Plywood Stacker Machine chain conveyor are unguarded and present an in-running nip point.  b.  Pallets of plywood roll into metal stops at the end of the Plywood Stacker Machine chain conveyor creating a pinch point where employees apply product labels.

29 CFR  1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and -2 on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced in their work area:  On or about May 21, 2024, employees involved in plywood processing at the Plystack, LLC facility located at 105 N Owen Ave, in Sutton, Nebraska were exposed to chemical exposure hazards in that employees did not receive training on how to access Safety Data Sheets for chemicals in use at their work site.
