A).      on or about 05/20/24, at the site of 133-28 150th Street. Jamaica, NY   Employees were exposed to crushed by hazards while digging an excavation 30 ft.  x 3 ft. x 4 ft. in depth for water proofing of the foundation wall. The concrete slab attached to the foundation wall was not supported or underpinned. A competent person did not inspect the excavation for hazardous conditions.
