29 CFR  1926.1153(c)(1):29 CFR  1926.1153(c)(1): For each employee engaged in a task identified in Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1:   Worksite located at 10430 E. Truman Road, Independence, Missouri- Employees were exposed to respirable silica hazards while the employee was cutting blocks with a radial saw. The employees were not protected from exposure to silica hazards by the use of  engineering practices such as a wet cutting method.

29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  Worksite located at 10430 E. Truman Road, independence, Missouri-Employee was exposed to a fall hazard while the employee was observed stepping down off the scaffold and on to a pallet elevated by the fork of a forklift. The pallet was not secured against displacement, nor was the pallet provided with adequate guardrails.
