29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn or attached to the boom or basket when working from an aerial lift:  (a) Jobsite at 1 Corporate Place in Middletown:  On or about 5/22/2024, the employer did not ensure that employees were protected from fall hazards of approximately 15 feet and greater while working from an aerial lift, without using a personal fall arrest or restraint system.
