(a) On or about May 23, 2024, and at times prior, located at 5817 46th Street, Kenosha, WI 53144, the employer had not tagged and/or removed from service an unstable portable job-made ladder with damaged siderails, thereby exposing employees to a fall hazard of approximately five (5) feet when accessing the magnet sorting conveyor line.
