On or about 5/23/2024, an employee utilizing an aerial lift was exposed to falls/contacting a lower level when climbing out of the aerial lift basket.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.   Location - Building 2, forth floor porch overhang.  On or about 5/23/2024, an employee utilizing an aerial lift was exposed to falls/contacting a lower level when climbing out of the aerial lift basket and attaching the lanyard to a non-approved anchor point.
