OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  a.) On or about May 24, 2024, employees performing powered industrial truck (forklift) operations were exposed to crushed-by and struck-by type hazards, while operating a Komatsu 50 model number F025-8 Serial number 134268, forklift that did not have an installed seat belt, or other means of restraint, to ensure the employees remain safely inside the operator's compartment in the event of a sudden, unexpected motion.
