29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  On or about May 24, 2024, at the worksite, 206 Fremont Street, Wayland, NY 14572:  Employees engaged in roofing activities on a 9:12 pitch roof were not protected from falling by use of guardrail systems, safety net systems, or personal fall arrest systems.  The employees were exposed to a fall hazard of 35 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about May 24, 2024, at the worksite, 206 Fremont Street, Wayland, NY 14572:  Employees engaged in roofing activities using a portable ladder to access the roof did not extend the ladder's side rails three feet above the access point.  Employees were exposed to fall hazards of 35 feet.
