29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Fieldale Farms Murrayville facility/ At Evisceration Department-Line 1 : On or about 5/28/2024, and at times prior, the employer exposed employees to eye hazards in that, the employer failed to ensure employees used appropriate eye protections while employees were performing sanitation activities and using caustic chemicals, such as but not limited to,  Lance - AFCO 5255 (corrosive), and AFCO 6008.

29 CFR  1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s).  a) Fieldale Farms Murrayville facility- At Evisceration Department : On or about 5/28/2024, and at times prior, the employer exposed employees to caught-in and struck-by hazards in that, the employer failed to ensure that LO/TO devices indicated the identity of the employees who applied the devices while performing sanitation and using LO/TO devices on line 1 bird washing machine.
