29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about May 23, 2024, employees engaged in shingle installation at heights greater than 6ft were not provided fall hazard recognition training. Employees were not provided the skills and knowledge necessary to mitigate, avoid, or eliminate fall hazards at the workplace.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about May 29, 2024, the employer did not provide employees engaged in roofing activities with ladder safety training. A portable extension ladder was used to access the walking/working surface that was not kept clear of debris around the bottom area exposing employees to a hazard likely to cause lacerations in the event of a fall from the ladder.

Employees were exposed to a fall hazard while performing shingle removal near the edge of the roof.
