29 CFR 1910.67(c)(2)(v): A personal fall arrest of travel restraint system that meet the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  On or about 5/28/2024, at 501 SE Becker Road, Port Saint Lucie, Florida, an employee engaged in tree trimming work was exposed to an approximated 25-foot fall hazard while working from an aerial lift without a means of fall protection such as a full body harness with a lanyard attached to the boom or basket.

29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  On or about 5/28/2024, at 501 SE Becker Road, Port Saint Lucie, Florida, the employer did not certified that a written hazard assessment was conducted to identify the needed personal protection equipment (PPE) to be used for the type of hazards encountered in the performance of tree trimming work.
