OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard.  On or about May 28, 2024, employees used a three leg chain sling to lift a concrete box that was designed with four lifting eyes which caused the load to be unstable while employees were inside the box.  Among other methods, feasible and acceptable means of abatement to correct these hazards include:  1) Follow the manufacturer's specifications for lifting the concrete drainage box.   2) Comply with ASME B30.9-2014 Slings, Section 9-1.10.2 Cautions to Personnel, (a) All portions of the human body shall be kept from between the sling and the load, and from between the sling and the hook, shackle, or other load handling device. (b) Personnel should not stand in line with or next to the leg(s) of a sling that is under tension.
