29 CFR 1926.452(y)(3): Surfaces on which stilts are used were not flat and free of pits, holes and obstructions, such as debris, as well as other tripping and falling hazards.  a) South Tower Floor 7, Room 769, on or about May 30, 2024,  employee was working on 62" stilts on a working surface with excessive debris such as drywall pieces, cords and hoses.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a) South Tower, Floor 7, Room 769: On May 30. 2024, an employee did not have the metal spreader bar fully extended on a 6-foot A-Frame ladder. He was standing on the 2nd to top step, facing away from the ladder, with one foot on the ladder and one foot on the frame of the wall.

29 CFR  1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  a) South Tower, Floor 7, Room 769: May 30, 2024; employees were using a drill which was connected to an extension cord that was missing the grounding plug.
