Located at the job site at 9531 Mitchell-Dewitt Road, Plain City, Ohio: On or about April 27, 2024, and at times prior, the employer did not ensure that an accident prevention plan was initiated and maintained to ensure that employees performing roofing work were protected including programs such as but not limited to; fall protection/prevention, ladder safety, and personal protective equipment. As a result, employees were exposed to fall and struck-by hazards.

As a result, employees were exposed to fall hazards of approximately 10 feet while performing roofing work.
