29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  On June 3, 2024, at the job site located at 913 N Odell Ave, Marshall Missouri 65340, employees were exposed to falls greater than 4 feet above the next lower level while unloading animal by product from a trailer without any fall protection. Employees were working on the top side of a hopper style over-the-road trailer to unload blood meal at a height measured approximately 9 feet to the ground.

An employee fatality occurred on or about June 3, 2024. The fatality was not reported to OSHA until June 4, 2024.
