On or about June 10, 2024 at 19801 Holland Road, Brook Park OH the employer failed to ensure employees operating Powered Industrial Vehicles, to include, but not limited to, Toyota Forklift Truck model 8FGCU25, Serial number 83948 traveled in a safe direction in that they travel with large metal totes forward with limited vision.
