29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.   a) Worksite: 652 E Main Street, Islip, NY 11751.  On or about 06/07/2024, Employee was installing plywood on the roof of a two-story residential building, approximately 28ft, above the ground. The employee was not protected by a means of fall protection and was exposed to falling to the ground below.    Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.   a) Worksite: 652 E Main Street, Islip, NY 11751.  On or about 06/07/2024, Employee was installing plywood on the roof of a two-story residential building, approximately 28ft, above the ground. The employee was not trained to recognize hazards associated with falls from heights.   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
