29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): On or about June 6, 2024, employees were exposed to fall hazards seven feet to a lower level when performing work activities from the Line 4 Dept. 437 Pour Car and the employer had not ensured a suitable fall protection system was used.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fire hazards: On or about June 6, 2024, the employer did not prevent blockages from occurring in Holder 8 thereby exposing employees working as Iron Pourers on Line 4 in Department 437 to the hazards associated with overpours and fires.
