This exposed employees to fall hazards of approximately nine (9) feet from the roof to the ground below.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  (a) On or about June 10, 2024, at a jobsite located at 1420 E Prairie View Dr Oak Creek, WI 53154, the ladder employees were using to access the roof of a one-story residential home did not extend three (3) feet above the upper landing surface, nor was the ladder secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. This exposed employees to a fall hazard of approximately nine (9) feet from the top of the ladder to the ground below.
