(a) On or about June 10, 2024, at 4482 Prairie Ridge Drive, Hubertus, WI 53033, the employer did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about June 10, 2024, at 4482 Prairie Ridge Drive, Hubertus, WI 53033, employees installing decking on an approximately 9:12 pitch roof were not wearing fall protection. This exposed employees to a fall hazard of approximately 11 feet from the roof to the ground below on the west (back) side of the home.

(a) On or about June 10, 2024, at 4482 Prairie Ridge Drive, Hubertus, WI 53033, the employer did not verify fall protection training by preparing a written certification record for employees tasked with installing decking on a new residential home.
