29 CFR 1926.150(a)(4): All firefighting equipment was not periodically inspected and maintained in operating condition.  Defective equipment was not immediately replaced:  a) Center of the Maintenance Bay of the Cryogenics Facility: A Buckeye portable ABC fire extinguisher was not removed from service due to the pin having been removed, there was no tag stating when the fire extinguisher was last inspected, the handle used to operate the fire extinguisher was bent and would not close properly and the needle was showing both green and red areas of the gauge showing the level of propellent was less then needed.
