29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. HMB Pleating Inc. 54-19 Myrtle Ave, Queens, NY, 11385 a) On or about June 7, 2024, the employer was notified of an employee's death on the same date but failed to report to OSHA within eight (8) hours of the fatality. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. HMB Pleating Inc. 54-19 Myrtle Ave, Queens, NY, 11385 a) On or about May 30, 2024, the employer failed to notify OSHA within 24 hours of a work-related incident that resulted in an employee hospitalization. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19
