29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about June 13, 2024, located at W148 N6890 Terriwood Drive, Menomonee Falls, WI 53051, the employer did not ensure that employees using pneumatic nail guns were wearing eye protection while installing shingles, exposing them to flying objects and struck-by hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about June 13, 2024, located at W148 N6890 Terriwood Drive, Menomonee Falls, WI 53051, the employer failed to ensure each employee tasked with staging bundles of shingles and conducting other roofing activities on a two-story residential roof were protected from falling to the lower level, exposing the employees to a fall hazard of approximately 18 feet to 23 feet.
