29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) On or about June 10 2024, at the jobsite located at N28W26940 Woodland Drive, Pewaukee WI, an employee was performing roofing work on two story residential house with a pitch of 8:12 with a fall height of approximately 10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.  (b) On or about June 10 2024, at the jobsite located at N28W26940 Woodland Drive, Pewaukee WI, an employee was performing roofing work on the roof of the porch of two story residential house with a pitch of 4:12 with a fall height of approximately 8 and 7/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about June 10 2024, at the jobsite located at N28W26940 Woodland Drive, Pewaukee WI, employees who were performing roofing work on a two story residential house without any fall protection in place did not have fall protection training to recognize fall hazards and how to implement fall protection measures.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about June 10 2024, at the jobsite located at N28W26940 Woodland Drive, Pewaukee WI, the ladder the employees used to access the roof of the residential building, did not extend three feet above the landing surface, exposing the employee to a fall hazard of approximately 8 7/10 feet.
