29 CFR  1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:   a.  On or about May 24, 2024, located at 7450 Wilson Blvd, Jacksonville, FL 32210, while working on a energized 480-Volt electrical panel, employees were exposed to electric shock and arc flash hazards in that the circuits had not been tested by a supervisor to verify that the equipment had been de-energized.

On or about May 24, 2024, located at 7450 Wilson Blvd, Jacksonville, FL 32210, while working on a energized 480-Volt electrical panel, employees were exposed to electric shock and arc flash hazards in that the circuits had not been tested by a supervisor to verify that the equipment had been de-energized.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a. At 7450 Wilson Blvd, Jacksonville, Florida 32210: On or about May 24, 2024, the employer failed to report a work-related in-patient hospitalization. The in-patient hospitalization occurred on May 24, 2024 and the employer notified OSHA on June 12, 2024.
