29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, an electrical outlet box (110 volt) being used to supply power to a portable fan (white) on the Sticker Stock table was being used as a pendant style device. The outlet box was laying on plastic that was covering a stack of books on the floor behind the Sticker Stock table. The outlet box lacked proper strain relief to be used as a pendant style device and was equipped with knockouts that could be displaced by an employee's fingers during use. Employees using the outlet box were exposed to the hazard of electrical shock. b) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, an electrical outlet box (110 volt) being used to supply power to a portable black fan (black) at the Sticker Stock table was being used as a pendant style device. The outlet box was laying on the floor behind the Sticker Stock table. The outlet box lacked proper strain relief to be used as a pendant style device and was equipped with knockouts that could be displaced by an employee's fingers during use. Employees using the outlet box were exposed to the hazard of electrical shock.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, an electrical cord that supplied power (110-volt) to both a white portable fan and a black portable fan on the Sticker Stock tables was badly damaged with large sections of the outer cover worn away and no strain relief where the cord entered each outlet box. Employees using the outlet boxes in Sticker Stock area were exposed to the hazards of electrical shock and electrocution.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures: a) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, an electrical outlet (110 Volt) located near the employee restrooms was not equipped with a cover. The outlet was being used to provide power to a ULine brand potable fan. The missing outlet box cover exposed employees working in the area to hazard of electric shock. b) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, a junction box located near the bottom of the stairs near the fire suppression system was not equipped with a cover. Employees working in the area where exposed to the hazard of electric shock due to the missing junction box cover. c) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, a junction box (110 Volt) mounted on a building column to the right of where the Sticker Stock table fans were plugged in was not equipped with a cover. The missing junction box cover exposed employees working in the area to hazard of electric shock. d) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, conduit leading into a junction box that was mounted above a row of storage racks in the main warehouse had separated from the junction box exposing the conductors housed within the conduit. The separated conduit exposed employees working in the area to hazard of electric shock.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, a flexible cord (110 Volt) used to supply power to a Frigidaire brand portable air conditioning system located inside the main office area was being used as a substitute for the fixed wiring of the structure. The flexible cord was wrapped around the outlet it was plugged in to, tied around a building beam above the outlet, and ran through an office window. b) Warehouse Area, 3000 7th Avenue, Altoona, PA 16602: Observed on or about June 11, 2024, a black flexible cord (110 Volt) was wrapped around a pipe and draped over the corner of the main office roof. The black flexible cord was used to supply power to a white portable fan on the sticker stock table.

The flexible cord was being used to provide power to a Frigidaire brand portable air conditioning system located inside the main office. Running a flexible cable through window opening could result in wear on the cord which could create fire and electric shock hazards.
