29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about June 17, 2024, during a roofing project for an awning on commercial building located at 6166 BROADVIEW RD, PARMA, OH 44134, the employer did not ensure employees working on a steep roof were protected from a fall of approximately 11 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 17, 2024, during a roofing project for an awning on commercial building located at 6166 BROADVIEW RD, PARMA, OH 44134, the employer did not ensure that the portable ladder an employees used to gain access to the roof, extended 3 feet above the roof (upper landing surface), exposing employees to falls hazards of approximately 11 feet.
