29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  Location - 1) East side of Bldg 27. 2) South side of Bldg 25.  On or about 6/17/2024, employees were exposed to fall hazards while working from a pump jack scaffold that was not erected under the supervision of a competent person.

Location - 1) East side of Bldg 27. 2) South side of Bldg 25.  On or about 6/17/2024, an employee on a pumpjack scaffold was not protected from falls to a lower level.

29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket.  Location - North end of Bldg 48.  On or about 6/17/2024, employees utilizing an aerial boom lift were not protected from falls out of the basket to a lower level. The employees were climbing onto the railing to gain access to a higher point.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under � 1926.502(e).  Location - North end of Bldg 48.  On or about 6/17/2024, employees utilizing an aerial boom lift were not protected from falls out of the basket to a lower level.
