29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: Note:  Exceptions are provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section:  a) On or about June 17, 2024, on the fifth level of the parking garage, where the scaffold was not capable of supporting its own weight and at least four times the maximum intended load while employees were cutting post tension tails and patching holes from the scaffold.

29 CFR 1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) On or about June 17, 2024, and times prior to, on the fifth level of the parking garage, where a competent person did not inspect the scaffold system prior to allowing employees to cut post tension tails and patching holes on the scaffold.
