29 CFR  1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  a) On or about May 18, 2024, the employer did not record a work-related injury from heat for employee 1 on the OSHA 300 Log of Work-Related Injuries and Illnesses form;  b) On or about May 18, 2024, the employer did not record a work-related illness from heat for employee 2 on the OSHA 300 Log of Work-Related Injuries and Illnesses form;  c) On or about May 26, 2024, the employer did not record a work-related illness from heat for employee 3 on the OSHA 300 Log of Work-Related Injuries and Illnesses form;  d) On or about May 27, 2024, the employer did not record a work-related illness from heat employee 4 on the OSHA 300 Log of Work-Related Injuries and Illnesses form;  e) On or about June 2, 2024, the employer did not record a work-related illness from heat employee 3 on the OSHA 300 Log of Work-Related Injuries and Illnesses form;  f) On or about June 8, 2024, the employer did not record a work-related illness from heat for employee 5 on the OSHA 300 Log of Work-Related Injuries and Illnesses form.
