29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  2250 Tilghman St. Allentown, Pa., Roof: On or about June 17, 2024, an employee was observed painting while standing on the pitched roof of residential structure, but no fall protection was in place, exposing the employee to an 11-foot fall hazard.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).   (a)  2250 Tilghman St. Allentown, Pa., Roof: On or about June 17, 2024, an employee used an extension ladder to access a pitched roof but the ladder was not used at a safe angle  of at least the one to four ratio, exposing the employee to a fall hazard
