29 CFR  1926.451(f)(14):Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  a) On or about June 21, 2024 at 7205 Waeti Dr.: an employee was allowed to utilize a bucket as a means to extend his reach while working on a tubular welded frame scaffold while installing flashing on the roof.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about June 21, 2024 at 7205 Waeti Dr.: an employee was exposed to a 12 feet 6 inch fall hazard while installing roof flashing, in that fall protection was not used.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about June 21, 2024 at 7205 Waeti Dr.: employees were exposed to a 12 ft 6 inch fall hazard, in that the ladder that did not extend at least 3 feet above the upper intended landing while accessing a ledge  12', 6" in height to install roof flashing.
