29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.   a) On or about June 5, 2024, at the above addressed jobsite, an employee was exposed to fall hazards while performing work from a Self-Riser Climbing Formwork System platform which was located more than 14 -inches away from the face of the work.

29 CFR 1926.451(e)(4)(i): A stair rail consisting of a top rail and a midrail was not provided on each side of each scaffold stairway.  a) On or about June 5, 2024, at the above addressed jobsite, the south core scaffold stairway accessed by an employee did not have a stair rail consisting of a top rail and a midrail, thereby exposing the employee to hazards associated with falls.
