29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6ft or more above lower levels were not protected by guardrail systems, safety net system, or a personal fall arrest system:  On or about June 21, 2024, employees engaged in residential roofing activities at 7926 S 75th Ave, LaVista, Nebraska were exposed to fall hazards while working on a roof with an eave height of 13ft in that the employer did not ensure the employees were using fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  On or about June 21, 2024, employees engaged in residential roofing activities at 7926 S 75th Ave, LaVista, Nebraska were exposed to fall hazards in that the employees used a ladder with side rails that did not not extend at least 3 feet past the upper landing surface.
