29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   On or about June 21, 2024, employees of Sierra Contractors Inc were engaged in roof replacement activities on a 4:12 pitch roof at 2911 Yaeger Road in Saint Louis, Missouri. The employer did not ensure employees on the roof utilized a fall protection system. The employees were working without any means of fall protection, exposing employees to a fall hazard of approximately 13 feet from gutter board to the ground.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:   On or about June 21, 2024, employees of Sierra Contractors Inc were performing roofing work at 2911 Yaeger Road in Saint Louis, Missouri. The employer did not ensure that employees using ladders do not carry objects or loads that could cause a fall. An employee was carrying a three-quarter inch, 4 feet wide by 8 feet long plywood sheet weighing approximately 60 pounds up an extension ladder.
