29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. Employees engaged in framing operations located at 5408 North 208th, Elkhorn, Nebraska 68022 were exposed to fall hazards in that employees were working off of platform elevated over 10ft above the ground below by a rough terrain forklift and the employer failed to install guard rails on the open side(s) of the platform as a means of fall protection. This was most recently documented on or about June 25th, 2024.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Employees engaged in framing operations on a residential construction jobsite located at 5408 North 208th, Elkhorn, Nebraska 68022 were exposed to fall hazards in that inadequately employees were observed working at heights greater than 6 feet and as high as 20 feet without fall protection. This was most recently documented on or about June 25th, 2024. Instances of inadequacy include but are not limited to: i. Employees failed to identify the 6-foot trigger height requiring fall protection in construction. ii. Employees failed to recognize the requirements for inspections of personal fall arrest equipment. iii. Employees failed to recognize the requirements for proper fit of personal fall arrest equipment . iv. Employees failed to recognize the requirements for stair rails on stairs with 4 or more risers.
