29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting concrete with a saw, without the use and wear of protective eyewear:  a. At a new residential construction site, located at, or near, 266 Tidgi Beach Ave. Bridgewater, St. Agustine, Florida:  On or about June 25, 2024, the employer exposed employees to struck-by hazards, in that protective eyewear was not utilized by employees while using an electrical saw and nail guns for framing work.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. At a new residential construction site, located at, or near, 266 Tidgi Beach Ave. Bridgewater, St. Augustine, Florida:  On or about June 25, 2024, the employer exposed employees to fall hazards greater than 6 feet to the unpaved ground below, in that fall protection was not utilized by employees while conducting framing work.
