A pump which was affixed to the wall at the rear of the facility was powered by an extension cord which was wrapped around a metal beam and plugged into an outlet connected to emergency lighting. 29 CFR 1910.305(a)(2)(xi):Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage. Support was not in the form of staples, cables ties, straps, or similar type fittings installed so as not to cause damage. At 100 Marion St, St Louis, MO, 63104, on June 26 and at times prior thereto employees were exposed to electrical hazards related to an extension cord which was in use to power a wall mounted pump. The cord was hanging from the outlet above an emergency light, dropping down and wrapping around a steel beam before being plugged into the wall mounted pump.
