Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.  On or about June 25, 2024, employee(s) were engaged in residential roofing activities at 451 West Sixth Street, Fremont, Nebraska. Employee(s) were exposed to fall hazards when roofing a residential building without using any form of fall protection at a height greater than 18 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about June 25, 2024, employee(s) were engaged in residential roofing activities at 451 West Sixth Street, Fremont, Nebraska. Employee(s) were exposed to fall hazards without training to recognize the hazards of falling and the procedures to be followed in order to minimize these fall hazards at a height.  Employee(s) were working at a height greater than 18 feet.
