29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or a personal fall arrest system:  On or about June 27, 2024, employees engaged in residential roofing activities at 6809 Beth Ave, Papillion, Nebraska were exposed to fall hazards while working on a roof with an eave height of 12ft in that the employer did not ensure the employees were using fall protection.

29 CFR  1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:   On or about June 27, 2024, employees engaged in residential roofing activities at 6809 Beth Ave, Papillion, Nebraska were exposed to fall hazards in that the employer failed to train employees to recognize the hazards associated with this work.  This should include training on mitigating those hazards through an acceptable method of fall protection.  Training should also include the requirements for fall protection systems and when it is appropriate to utilize those systems.
