Jobsite: On or about June 19, 2024, the employer did not ensure that a competent person was on site to conduct daily inspections for evidence of a situation that could result in possible cave-in, hazardous atmospheres, or other hazardous conditions to include but not limited to overseeing the excavation operations such as spoil pile placement, ladder selection and usage.
